GUIDE: A new standard for dementia care.

At Home Harmony is an active participant in the Centers for Medicare & Medicaid Services’ GUIDE model. GUIDE’s goal is greater care coordination and additional services to achieve better outcomes, reduce hospitalizations and emergency room visits, and treat the whole person’s physical, mental, emotional, and spiritual needs.

The GUIDE program can be a standalone service at your community, powered by At Home Harmony. The program requires very little of your staff – we only need a referral/introduction to the families you’d like us to help. Then, we take it from there.
