She finds fulfillment in changing lives, bringing peace of mind to families, and improving the quality of life for aging adults.

Arielle’s background includes 8 years in a medical surgical ICU and 13 years as an NP in the acute care and long-term care arena. She is passionate about dementia care and works to ease the burden of those living with dementia and their caregivers through the expansion of Medicare’s GUIDE program. She believes in holistic healthcare that meets individuals where they are within life’s journey. Arielle enjoys spending time with her husband and two children; she is an animal lover who is passionate about gardening, travel, and enjoys live music.

With over a decade of nursing experience, Rachel serves as Director of Clinical Operations, leading the Care Concierge team. From one end of Broad Street to the other, she understands the unique resources available and is always eager to connect patients with the tools they need to thrive.

Caroline’s nursing career began in the Adult Emergency Department at VCU Health, where she gained invaluable experience while completing her studies to become an adult-gerontology acute care nurse practitioner. After graduation, she honed her expertise in pulmonology and sleep medicine, followed by a stint in primary care, further broadening her skill set. Outside of work, Caroline enjoys spending time with her husband, daughter, and their puppy. They love traveling near and far, cooking, and being outside.

Crystal started her health career as a cardiology tech and was afforded many opportunities to learn while she finished her nursing degree. She worked in emergency medicine then decided to pursue further studies. After graduating, she has focused on urgent care, nephrology and geriatrics. The aspect she loves most about her job is being able to teach. She relishes being able to take the time to explain changes that come with aging.

Darli Aung
Darli brings a specialty in primary care geriatrics and relishes delivering patient-centered care to older patients based on their personal medical history and unique needs. With experience in a variety of healthcare settings, Darli has provided comprehensive medical care to patient with complex, acute, chronic, and critical health issues, including stroke, brain and spinal cord injury, cancer, and neurodegenerative disorders. Most recently, she cared for patients at Heartland Hospice and Abode Care Partners, performing assessments, advanced care planning, and comfort visits, as well as reviewing treatment plans and medications.

Olivia has worked in healthcare since 2009, when she became a nurse assistant in the hospital while going through nursing school. After working as an oncology nurse, she earned her nurse practitioner degree in 2015. She has worked in family practice, retail health, and telemedicine. Her passion is learning what matters to her patients and their families, making their plan of care fit their vision. Outside of work, Olivia loves to have fun with her husband and young daughter, perform in and choreograph for musical theater, and go on nature walks.
