Do we need to give up our existing doctor?
No. We collaborate with many primary care doctors and specialists. We also can serve as your primary care provider.
Who will visit my home?
Typically a nurse practitioner (NP) or physician assistant (PA). These are advanced healthcare practitioners with significant training, who are overseen remotely by a physician.
We are a local company of geriatric specialists who, in answer to Medicare’s efforts to keep aging seniors healthy and out of the hospital, have built a collaborative model to address the needs of the aging adult.
Our coordinated, preventive approach to care is what sets us apart from the disjointed care everyone has grown to accept. Our clinicians and physicians work together to get to know you and provide care in your home where you are safest.
When was the last time your provider spent more than 10-15 minutes with you? Our first assessments take 60-90 minutes, because it’s important that we get to know you, review all of your medications, understand how you’re caring for yourself, and can help you best carry out your health and wellness goals to stay healthy.
Will a doctor ever come to my home?
Our practice is overseen by a physician with vast experience with the senior population. Our provider group who practice under the physician include highly trained and experienced geriatric PAs and NPs.

Currently, in the Central Virginia area.
Do you accept Medicare?
Yes. All our medical and pharmacy services are covered by Medicare. We participate with all major Medicare Advantage and Prescription Drug Plans and commercial plans other than Caremore for in-home primary care services.
What does in-home medical care include?
Think of us as a geriatric specialist who provides primary care in the home. We provide the following:
- Preventive and Wellness visits
- Sick Visits
- Coordination and support for Chronic Medical Conditions
- Cognitive Assessments
- Therapy Referrals, such as Physical Therapy
- In-Home Lab Work
When I call to learn more about or sign up for Harmony's services, what should I expect?
An Account Manager will walk you through the steps to ensure you are a qualified candidate for our care model. Once that has been confirmed, the Account Manager can perform all intake over the phone and get you scheduled for your first assessment! Plan to have your Medicare/Insurance cards ready.
What should I expect after scheduling the first assessment?
When your appointment is upcoming, you will receive a call from our team to confirm your appointment. Add 804-210-3103 to your contacts, this is the number from which you’ll receive the call.
Why does it take so long to schedule the first assessment? Will follow-ups be the same?
The first assessment is a long, in-depth appointment, so we are more limited in our schedule space. After you have this initial assessment, you will have follow-ups as often as you need. If you are sick, you will be able to contact our office and schedule triage much faster than the scheduling was for your initial appointment.
What should I expect at my first appointment?
A provider will come to your home and conduct your first appointment. This compassionate provider takes the time to get to know you and establish care to develop your Harmony plan. Then, you are considered an enrolled Harmony patient, and our providers will walk you through your Harmony plan of care with appropriate follow-ups and assorted services of ours to keep you as healthy as possible.
I have Power of Attorney (POA) or am a caregiver. Should I be present for the first assessment?
Yes, you should be present for the first assessment and as many subsequent visits as you are able to make. It is especially important as we establish care that we understand the full health history and challenges of our patient, and caregivers are generally instrumental in understanding the full picture so we can best develop a care plan to address all patient needs.
Is At Home Harmony certified home health?
No, we do not provide certified home health services. Think of us as a home-based primary care provider who takes care of you from many angles with our ongoing primary medical care, pharmacy, chronic care management, monitoring, and personal care. Certified Home Health providers, instead, provide temporary skilled services (skilled nursing care, physical therapy, occupational therapy, speech-language pathology services, wound care, and more) to you when you are recovering from an acute episode or illness.
We partner with many Home Health agencies to provide you with their services when you need it.

Deborah, 72, was hospitalized for acute kidney failure and dehydration. She also has a medical history of diabetes, hypertension, depression, mild cognitive impairment, and chronic low back pain. Patient had not seen a primary care provider in more than 2 years due to transportation difficulties.

Juan and Carmen Mendoza
Juan, 84, has atrial fibrillation, moderate cognitive impairment, COPD, and is unable to perform some basic ADLs alone. Carmen, 82, was the caregiver for Juan until she had a fall and broke her right hip. She has been in a post-acute rehab (discharging soon), and Juan has been temporarily cared for by daughters and neighbors. His daughters live in a different state with their own families. Juan and Carmen have not been to a primary care provider in over a year and the next available appointment is in 4 months. Transportation is a challenge.
