Dr. Gilstad is dedicated to clinical excellence and innovation in the care of older adults with complex needs including dementia. A retired U.S. Navy Captain, he held key roles including Commanding Officer of Naval Medical Research Unit 3 and Director of the Armed Forces Radiobiology Research Institute. Dr. Gilstad earned his medical degree from the Uniformed Services University, completed residency at National Naval Medical Center and fellowship at Johns Hopkins. He is a board-certified in internal medicine and geriatrics.

With a master’s degree in occupational therapy and undergraduate degrees in health sciences and psychology, Jennifer is responsible for overseeing our coordinated care team. She has years of experience caring for older adults in the home with a goal of helping every person maintain their independence and a high quality of life. A certified lymphedema therapist, Jennifer and her husband have four children and are devoted Philly sports fans.

Mark began his healthcare career in 1991 and has built an expertise in healthcare operations, and pharmacy benefit management. His career has focused on coordinating the disparate elements of healthcare to create a simplified and effective care model for patients. A Boston native, Mark is a graduate of the University of Vermont and lives in Richmond with his wife. They have two grown daughters. He enjoys golf, travel, music, and standup paddleboarding.

With over a decade of nursing experience, Rachel serves as Director of Clinical Operations, leading the Care Concierge team. She partners with patients and caregivers to deliver coordinated case management and medical support. A Richmond native, Rachel combines her deep knowledge of the local community with her expertise in navigating the healthcare system. From one end of Broad Street to the other, she understands the unique resources available and is always eager to connect patients with the tools they need to thrive.

Prior to founding At Home Harmony, Will owned and operated a company that created Medicare Advantage plans for residents in long-term care nursing homes. Will is driven to create innovative solutions to help seniors with complex conditions and transform the quality of geriatric care nationwide. He resides in Richmond with his wife and has four daughters.

Dr. Smith completed his residency in Internal Medicine at Parkland Hospital in Dallas. He is board certified in Internal Medicine and Critical Care with 35+ years of experience in private practice and academic medicine, including extensive work in diabetes management, internal medicine, and critical care. He has over two decades of teaching experience as an Associate Clinical Professor, mentoring medical students and residents. He lives in Richmond and enjoys spending his free time visiting his children and grandchildren.

Caroline’s nursing career began in the Adult Emergency Department at VCU Health, where she gained invaluable experience while completing her studies to become an adult-gerontology acute care nurse practitioner. After graduation, she honed her expertise in pulmonology and sleep medicine, followed by a stint in primary care, further broadening her skill set. Outside of work, Caroline enjoys spending time with her husband, daughter, and their puppy. They love traveling near and far, cooking, and being outside.

Crystal started her health career as a cardiology tech and was afforded many opportunities to learn while she finished her nursing degree. She worked in emergency medicine then decided to pursue further studies. After graduating, she has focused on urgent care, nephrology and geriatrics. The aspect she loves most about her job is being able to teach. She relishes being able to take the time to explain changes that come with aging.

Jordan began her career as a bedside Nurse at VCU Medical Center before earning her Nurse Practitioner degree and transitioning to work with older adults in the Primary Care Provider role. With a background in managing chronic illness, functional decline, and cognitive impairment, she is dedicated to providing comprehensive, compassionate, and evidence-based care. She values a collaborative approach and works closely with patients and families to understand their priorities to promote independence and dignity in aging.

Sarah Allen is a board-certified Family Nurse Practitioner with clinical experience spanning family medicine, geriatrics, cardiology, and aesthetic medicine. She began her career as an inpatient nurse at the University of Virginia before earning her nurse practitioner credentials in 2021. She is a proud UVA fan who met her husband in college — they are now parents to an 18-month-old son, with another little one on the way. When she’s not caring for patients or family, Sarah enjoys the outdoors and tending to the family’s backyard chickens.

Ashley is an Adult-Gerontology Acute Care Nurse Practitioner with a strong foundation in critical care. She began her career at VCU Health as a Patient Care Tech before advancing to Registered Nurse in the neuroscience ICU. As an NP, she has worked with Internal Medicine and Palliative Care teams at Select Specialty Hospital, and with Rebirth Advanced Healing following her wound care and debridement certification. She enjoys reading, gardening, hiking with her dogs, and visiting her large Italian family in New York.

Johnee Nedrick is a board-certified Family Nurse Practitioner with over a decade of healthcare experience, including eight years as a registered nurse across medical-surgical, rehabilitation, public health, and gastroenterology settings. As an NP, she has specialized in long-term care and weight loss medicine. Rooted in a large, close-knit family, Johnee brings a deep sense of community to both her personal and professional life. Outside of work, she enjoys event planning and world travel.
